Q: What Documents are needed for certication?
A: SUBMISSION OF APPLICATION: Initial Application for Examination
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Completed Application
    2. Two 2 ½ x 2 ½ Photo (Must be signed and dated on the back)
    3. Copy of Photo ID, Passport or certified copy of birth certificate
    4. Three original letters of reference of good moral character addressed to the Board containing complete legal name of the individual making the reference, with his/her mailing address, residential address, place of employment and telephone numbers. Original letters must be signed.
    5. Police Clearance (from all places of residence within the last 12 months)
    6. Payment fee of $20.00 (All payments must be paid at Treasure of Guam unless application was mailed in from off-island.)
    7. Transcripts (Must reflect the number of hours the applicant is applying for Cosmetology=1600 Hrs, Barber=1600, Manicurist=400 Hrs, Esthetician=600 Hrs. of 1300 Hrs.)
    8. If applicant graduated out of the country must submit request for “General Evaluation from the following:

      Upon completion of evaluation, Aequo International will send the report directly to the Board.

  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Completed Application
    2. Two 2 ½ x 2 ½ Photo (Must be signed and dated on the back)
    3. Payment Fee of $10.00
    4. Police Clearance (Within the last 12 months)
    5. Money Order for NIC
      1. Written & Practical - $38.00
      2. Written Only - $18.00
      3. Practical Only - $20.00
    6. When application is complete with all required documents attached add name to agenda.


SUBMISSION OF APPLICATION: Application for Apprenticeship
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Completed Application
    2. Two 2 ½ x 2 ½ Photo (Must be signed and dated on the back)
    3. Copy of Photo ID, Passport or certified copy of birth certificate
    4. Three original letters of reference of good moral character addressed to the Board containing complete legal name of the individual making the reference, with his/her mailing address, residential address, place of employment and telephone numbers. Original letters must be signed.
    5. Police Clearance (from all places of residence within the last 12 months)
    6. Agreement to Supervise (GBOC-11)
    7. Payment Fee of $20.00
    8. Must be over sixteen(16) years of age
    9. Completed technical instruction, a minimum of 1,300 hours in barbering or 1,300 hours in cosmetology, and a minimum number of practical operations for each subject as specified in Board regulations for course taught in schools approved by the board.
    10. When application is complete with all required documents attached add name to agenda.


SUBMISSION OF APPLICATION: Renewal Application
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Complete entire Application and attach expiring or expired license.
    2. Two 2 ½ x 2 ½ size pictures take within the last 60 days (white background only, print name, sign signature and date at the back of photo)
    3. Continuing Education Report must be fully completed and signed. (Certificate of Completion must be attached with CE form.
    4. Payment Fee of $4.00


SUBMISSION OF APPLICATION: Establishment Application
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Specify by checking one of the following boxes
      1. Change Owner of Establishment
      2. Change Operator of Establishment
      3. Name Change on Establishment
      4. New Establishment
      5. Change of Location for Establishment
    2. Provide the name of the Establishment, Owner of the Establishment, Old Location of Establishment (if Applicable) or New Location of Establishment.
    3. Submit a copy of ALL Licensees working under the Establishment
    4. Copy of Sanitary Inspection (New Applicant)
    5. Copy of Business License
    6. Payment Fee of $20.00


SUBMISSION OF APPLICATION: Renewal Establishment Application
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Complete Application
    2. Submit a copy of current Business License
    3. Copies of ALL employee’s license working in the Establishment
    4. Payment Fee of $4.00


SUBMISSION OF APPLICATION: Reciprocity Application
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Complete and notarize Application
    2. Two 2 ½ x 2 ½ size pictures take within the last 90 days (white background only, print name, sign signature and date at the back of photo)
    3. Copy of driver’s license, U.S. Passport or a certified copy of birth certificate
    4. Three (3) letters of reference of good moral character addressed to the Board containing the complete legal name of the individual making the reference, with his/her mailing address, residential address, place of employment and telephone numbers.
    5. Police clearance from your place of residence within the last 12 months.
    6. Payment fee of $20.00
    7. Verification from Original State of License
    8. Copy of current License

SUBMISSION OF APPLICATION: Special Japanese License Application
  1. Client calls to make an appointment
  2. Upon receiving Application stamp with the date it was received on
  3. Follow check list provided on the front of the application
    1. Complete and notarize Application
    2. Two 2 ½ x 2 ½ Photo (White background only, print name, sign signature and date at the back of photo)
    3. Copy of driver’s license, U.S. Passport or a certified copy of birth certificate
    4. Three (3) letters of reference of good mornal character addressed to the Board containing the complete legal name of the individual making the reference, with his/her mailing address, residential address, place of employment and telephone numbers.
    5. Police Clearance
    6. Payment Fee of $200.00 for Initial Application and $800.00 for Annual Special License
    7. Must be over sixteen (16) years of age
    8. Complete technical instruction, a minimum of 1600 hours in a school term of at least none (9) months, detailing the subjects and hours of training For out of country graduates: Request for “General Evaluation” from the following:
